From Exceptional Cases to Enduring Wisdom, From Broca to the Bedside: The Renaissance of the Case Report in Neuropsychiatry Through NPCR 3 1

Abstract

This comprehensive academic review provides an extensive examination of Neuropsychiatry Case Reports (NPCR), a newly established peer-reviewed, open-access journal dedicated to the publication of clinically significant, unusual, rare, and challenging cases at the intersection of neurology, psychiatry, psychology, and neuroscience. The review situates NPCR within the broader intellectual and clinical history of the case report, tracing its evolution from the foundational narratives of 19th-century neurology and psychiatry to its contemporary role in an era of evidence-based medicine, precision therapeutics, and complex neuropsychiatric disorders. It argues that NPCR addresses a critical and increasingly marginalised niche in the scholarly landscape: the systematic documentation and dissemination of exceptional clinical experiences that provide unique insights into disease mechanisms, diagnostic processes, treatment responses, and the lived reality of neuropsychiatric illness. Through detailed examination of landmark case reports—including Phineas Gage, Broca's aphasia, Alzheimer's first case, the amnesic patient HM, and contemporary autoimmune encephalitis—the review demonstrates how extraordinary cases have fundamentally shaped our understanding of brain-behavior relationships. The review assesses the journal's clinical epistemology, its commitment to bridging the historical divide between neurology and psychiatry, its role in the education of clinicians, its place within the broader evidence hierarchy, and its potential to catalyse new research directions.
